Abstract
Objective To investigate the combined predictive value of serum neutrophil gelatinase-associated lipocalin (NGAL), procalcitonin (PCT), and interleukin-6 (IL-6) levels for catheter-related infection (CRI) in uremic patients undergoing maintenance hemodialysis (MHD). Methods A total of 150 uremic patients receiving MHD at Nanyang Central Hospital between August 2022 and August 2025 were enrolled in this study. Patients were divided into an infection group (n=32) and a non-infection group (n=118) based on the presence or absence of CRI. Serum levels of NGAL, PCT, and IL-6 were measured. Multivariate logistic regression analysis was used to identify factors influencing CRI in MHD patients. Receiver operating characteristic (ROC) curves were constructed to evaluate the predictive performance of serum NGAL, PCT, and IL-6 levels for CRI. Result The proportion of diabetes mellitus, catheterization time, NGAL, PCT and IL-6 levels in the infection group were higher than those in the non-infection group (χ2/t=4.476, 3.330, 6.932, 7.467 and 7.983 respectively; P=0.034, 0.001, <0.001, <0.001 and <0.001 respectively). Serum NGAL, PCT and IL-6 were the risk factors for CRI in uremic MHD patients (OR=2.064, 2.716 and 2.192 respectively; 95% CI:1.153~3.694, 1.275~5.788 and 1.119~4.293 respectively; P=0.015, 0.010 and 0.022 respectively). The AUC of serum NGAL, PCT and IL-6 for predicting CRI in uremic MHD patients were 0.771 (95% CI:0.680~0.862), 0.851 (95% CI:0.777~0.925) and 0.819 (95% CI:0.724~0.913) respectively. The AUC of combined serum NGAL, PCT and IL-6 levels for predicting CRI in uremic MHD patients was 0.926 (95% CI:0.867~0.985), better than that of individual level for prediction (Z combined-NGAL=3.864, P=0.001; Z combined-PCT=2.327, P=0.020; Z combined-IL-6=2.495, P=0.013). C onclusion Elevated NGAL, PCT, and IL-6 levels are closely related to an increased risk of CRI in uremic MHD patients. The combined use of the three serum levels predicts CRI better in uremic MHD patients.
