授权管理在连续性肾脏替代治疗专科护士培训中的应用研究

摘要: 【摘要】目的探讨授权管理在连续性肾脏替代治疗(continuous renal replacement therapy, CRRT)专科护士中的应用效果。方法在武汉大学人民医院开展CRRT 专科护士培训,并对通过考核的49 名专科护士进行授权和管理。比较培训前后护士的结构授权水平、心理授权水平、工作满意度水平和突发事件处理能力。结果与培训前相比,培训后CRRT 专科护士的结构授权水平、心理授权水平和工作满意度均明显提升(t 值分别为4.838,2.362,2.484;P 值分别为<0.001,0.022,0.017),其中心理授权水平仅工作影响维度得分与培训前无显著性差异(t=0.259,P=0.797),工作满意度量表仅外部满意度分量表的得分与培训前无显著性差异(t=1.899, P=0.064),CRRT 专科护士突发事件处理能力较培训前得到增强(χ2=4.047,P=0.044)。结论授权管理结合CRRT 专科护士培训可有效提升护士的结构授权水平、心理授权水平、工作满意度及专科护理能力,降低CRRT操作过程中不良事件的发生率,具有一定推广价值。

Abstract: 【Abstract】Objective To explore the application of empowerment management in the training of nurses specialized in continuous renal replacement therapy (CRRT). Methods The training of nurses specialized in CRRT was carried out in Hubei General Hospital. Empowerment management was performed in the 49 CRRT specialized nurses passed the examination. Their structural empowerment, psychological empowerment, job satisfaction and emergency handling ability were compared before and after training. Results The scores of structural empowerment (t=4.838, P<0.001), psychological empowerment (t=2.362, P=0.022) and job satisfaction (t=2.484, P=0.017) increased significantly after training as compared with those before training. However, the impact dimension of psychological empowerment (t=0.259, P=0.797) and the external satisfaction subscale of job satisfaction (t=1.899, P=0.064) showed no significant differences after training as compared with those before training. The emergency handling ability of CRRT specialized nurses improved significantly (X2=4.047, P=0.044). Conclusion Empowerment management combined with training of nurses specialized in CRRT can effectively increase their structural empowerment, psychological empowerment, job satisfaction, and specialized nursing ability levels, and decrease the adverse events during CRRT operation. Therefore, this method is worthwhile to be used in clinical practice.
