维持性血液透析患者焦虑、抑郁情况及与生活质量的关系

The effect of depression and anxiety on quality of life in maintenance hemodialysis patients

摘要: 【摘要】目的探讨维持性血液透析(maintenance hemodialysis,MHD)患者焦虑、抑郁患病率及与生活质量的关系。方法采用焦虑自评量表(self rating anxiety scale,SAS)、抑郁自评量表(self rating depression scale,SDS)和肾病及生活质量量表(KDQOL-SFTM1.3 调查量表)对2017 年1 月~2017年12 月在四川省资阳市人民医院血液透析中心进行MHD 治疗患者的焦虑、抑郁及生活质量进行横断面调查分析。结果①共纳入124 名MHD 患者,焦虑患病率16.13%,抑郁患病率36.29%,焦虑合并抑郁患病率10.48%;②MHD 无焦虑/抑郁患者KDQOL-SFTM量表大部分维度评分明显高于焦虑/抑郁患者;而焦虑合并抑郁患者大部分维度评分均低于无焦虑无抑郁,单纯焦虑/抑郁患者;③MHD 患者SAS、SDS 评分与KDTA 总分呈明显负相关(P 值分别<0.001, <0.001)、与SF-36 总分呈负相关(P 值分别为0.003, 0.001)。结论四川省资阳市人民医院血液透析中心MHD 患者焦虑患病率16.13%,抑郁患病率36.29%,同时合并焦虑和抑郁患病率10.48%,焦虑、抑郁在不同方面、不同程度影响MHD 患者生活质量,且焦虑合并抑郁时更明显。

Abstract: 【Abstract】Objective To investigate the prevalence of anxiety and depression and their effects on quality of life in maintenance hemodialysis (MHD) patients. Methods MHD patients treated in the Dialysis Unit of Sichuan Provincial Ziyang People’s Hospital from Jan. 2017 to Dec. 2017 were enrolled in this cross-sectional study. Self-rating Depression Scale (SDS), Self-rating Anxiety Scale (SAS) and KDQOL-SFTM Questionnaire were used to assess anxiety, depression and quality of life in MHD patients. Results ①A total of 124 MHD patients were enrolled in this study. The prevalence of anxiety, depression and comorbid depression and anxiety were 16.13%, 36.29% and 10.48% respectively. ②The MHD patients without anxiety/depression had better performance than those with anxiety/depression (P<0.05), and those with comorbid depression and anxiety had lower performance than those without anxiety, depression or comorbid depression and anxiety. ③The scores of SAS and SDS were negative correlated with KDTA score and SF-36 score (P<0.05). Conclusion The prevalence of anxiety, depression and comorbid anxiety and depression were 16.13%, 36.29% and 10.48% respectively in the MHD patients treated in the hemodialysis center of Sichuan Provincial Ziyang People’s Hospital. Anxiety and depression impair the quality of life in MHD patients, especially in those with comorbid anxiety and depression.
