国产血液滤过器对血液透析滤过治疗血清β2微球蛋白的清除效果

Removal of β2- microglobulin during on- line hemodiafiltration using a domestic hemofilter

摘要: 目的探讨国产血液滤过器对血液透析滤过治疗血清β2微球蛋白(β2-microglobulin,β2-MG)的清除效果。方法选择30 例病情稳定且血清β2微球蛋白在正常上限10 倍以上的维持血液透析患者进行自身对照研究,分别用国产血液滤过器(朗生,膜面积1.4m2)和进口血液滤过器(东丽,膜面积1.6m2)进行在线血液透析滤过(0n-line hemodiafiltration,On-line HDF)治疗。比较两者血液透析滤过前后血清β2微球蛋白的下降率的差异。结果国产(朗生)血液滤过器组血β2微球蛋白下降率(71.729± 7.393)%,进口(东丽)血液滤过器组血β2微球蛋白下降率为(74.707±7.889)%,两者没有统计学差异(t=1.508,P 值=0.137)。单位膜面积朗生血液滤过器β2微球蛋白下降率为(51.235±5.280)%,东丽血液滤过器β2 微球蛋白下降率为(46.692±4.930)%,有统计学差异(t 值=-3.445,P=0.001)。结论国产朗生血液滤过器采用后置换进行在线血液透析滤过时,对β2微球蛋白有良好的清除效果。

Abstract: Objective To investigate the removal of serum β2-microglobulin during on-line hemodiafiltration using a domestic hemofilter. Methods In this single center, self-control, open-label clinical study, 30stable hemodialysis patients whose serum β2-microglobulin increased to more than 10 times above the upper
normal limit were chosen to receive post- dilution online hemodiafiltration using a domestic hemofilter(Lengthen, membrane area 1.4m2) or an imported hemofilter (Toray, membrane area 1.6m2). Reduction ratio (RR) of β2-microglobulin was assessed. Results The RR of serum ß2- microglobulin was similar after online hemodiafiltration using a Lengthen (71.729%± 7.393%) hemofilter and a Toray hemofiter (74.707%±7.889%, t=1.508, P=0.137). Conclusion There was a good removal effectiveness of ß2-microglobulin when the domestic hemofilter was used in post-dilution online hemodiafiltration.
