核心稳定训练对维持性血液透析患者运动能力、透析充分性和生活质量的影响

Effects of core stabilization training on exercise capacity, dialysis adequacy and quality of life in maintenance hemodialysis patients

摘要: 目的  探讨核心稳定训练对维持性血液透析(maintenance hemodialysis,MHD)患者运动能力、透析充分性、疲乏症状和生活质量的影响。 方法  选取2023年11月─2024年5月在上海交通大学医学院附属第六人民医院血液透析中心进行MHD的患者60例为研究对象,采用随机数字表法分为试验组(n=29)和对照组(n=31)。对照组实施常规治疗和护理,试验组在常规治疗和护理的基础上进行核心稳定训练。比较2组干预前后尿素清除指数(Kt/V)、尿素下降率(urea reductionrate,URR)、6 min步行试验(6 minutes walk test,6MWT)、30s坐立试验(30-second sit-to-stand test,30s-STS)、疲乏量表-14(fatigue scale-14,FS-14)得分、健康调查量表(the 36-item short form health survey,SF-36)得分的差异。 结果 干预后试验组患者Kt/V、URR、6MWT、30s-STS、SF-36得分高于对照组(t=7.271、2.758、7.072、11.905、3.276,P<0.001、0.021、<0.001、<0.001、0.002),FS-14得分低于对照组(t=-5.647,P<0.001);核心稳定训练后,试验组Kt/V、URR、6MWT、30s-STS、SF-36得分较基线水平升高(t=-6.558、5.289、-14.028、-25.713、-12.371,均P<0.001),FS-14得分较基线水平降低(t=-3.251,P<0.001)。 结论  透析中核心稳定训练可以改善MHD患者透析充分性,增强运动能力,减轻疲乏症状,提高生活质量。

Abstract: Objective   To explore the effects of core stabilization training on exercise ability, dialysis adequacy, fatigue symptoms and quality of life in patients with maintenance hemodialysis (MHD).   Methods  Sixty patients undergoing MHD in the Hemodialysis Center of The Sixth People's Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from November 2023 to May 2024 were selected as the study objects. They were divided into the experimental group (29 cases) and the control group (31 cases) by random number table method. The control group received conventional treatment and nursing, and the experimental group received core stabilization training on the basis of conventional treatment and nursing. The differences of urea clearance index (Kt/V ), urea reduction rate (URR), 30 second sit to stand test (30s STS), 6-minute walk test (6MWT), fatigue scale-14 (FS-14) scores, and the 36 item short form health survey (SF-36) scores were compared between the two groups before and after the intervention.  Results   After core stability training, the scores of Kt/V, URR, 6MWT, 30s-STS, FS-14, and SF-36 in the experimental group were significantly different from the baseline levels (t =- 6.558, 5.289, -14.028, -25.713, -3.251 and -12.371 respectively; all P<0.001) and significantly different from those in the control group (t =7.271, 2.758, 7.072, 11.905, -5.647 and 3.276 respectively; P<0.001, 0.021, < 0.001, < 0.001, < 0.001 and 0.002 respectively).  Conclusions   Core stabilization training during dialysis can improve dialysis adequacy, enhance exercise ability, reduce fatigue symptoms, and improve quality of life in MHD patients.
