摘要

妊娠期高脂血症性重症急性胰腺炎(hyperlipidemiainduced severe acute pancreatitis,HLSAP)是危及母儿生命的严重并发症,本文报道1例22岁孕29周HLSAP患者,血清三酰甘油(triglyceride,TG)达78.65 mmol/L。经多学科团队评估,入院24 h内启动双重滤过血浆置换(double filtration plasmapheresis,DFPP)。2次治疗后,TG降至21.65 mmol/L(降幅72.5%),腹痛24 h内控制,5 d转出重症监护室,11 d出院,孕38周剖宫产,母婴结局良好。早期DFPP治疗可快速降低血脂及炎症介质,是保障母婴安全的重要策略。

Abstract

Gestational hyperlipidemia-induced severe acute pancreatitis (HLSAP) is a life-threatening complication. We report a 22-year-old pregnant woman (gestation of 29 weeks) with HLSAP with serum triglyceride (TG) of 78.65 mmol/L. Double filtration plasmapheresis (DFPP) was initiated within 24 hours. After two sessions, TG decreased to 21.65 mmol/L (72.5% reduction) with resolved abdominal pain within 24 hours. She was discharged on day 11 and delivered a healthy infant via cesarean section at 38 gestational weeks. Early DFPP effectively clears lipids and inflammatory mediators, ensuring maternal-fetal safety.
