不同透析膜对维持性血液透析患者急性时相蛋白的影响
【摘要】目的 观察不同透析膜在血液透析患者炎性反应中的作用。方法 10例MHD患者通过自身对照,先后给予醋酸纤维膜(CA)、聚砜膜(PS)和血仿膜(H)进行透析治疗。患者分为CA组、PS组、H组,检测透析前、后血清CRP、CER、AAG和SAA水平。结果 透析前各组血清CRP、CER、AAG、SAA水平间差异均不显著。透析后,各组血清CRP、CER显著升高(P<0.05),H组AAG水平显著降低(P<0.01),但透后三组间CRP、 CER、 AAG、 SAA增高或降低值差异均不显著。结论 透析过程血液与各种人工透析膜接触均可导致患者炎症反应,CA膜、PS膜、H膜在透析过程对患者的炎症反应无明显差异,四种急性时相反应蛋白(APP)在评价透析膜对患者炎症刺激中,以CRP和CER敏感和迅速。
【Abstract】Objective To investigate the effect of different dialyzer membranes on the inflammatory reaction of end- renal failure patients with maintaining hemodialysis (MHD). methods Ten MHD patients who were randomized in selfcontrolled study were dialysed with cellulose acetate(CA), Polysulfone(PS) or hemophane(H) as dialysis membrane. Those patients were divided into three groups including CA, PS, H. the serum C reaction Protein (CRP), ceruloplasmin(CER), α1- acid glycoprotein (AAG), serum amyloid A (SAA) were measured by routine methods. Results There were no significant differences on predialysis acute-phase protein (APP) parameters among three groups. After dialyzing, serum CRP and CER increased markedly(P<0.05) among three groups, and serum AAG was decreased markedly in group H(P<0.01), but the increments or decrements of APP were on significant differences in three groups (P>0.05). Conclusion During hemodialysis, the inflammatory reaction is led to by different hemodialysis membranes, and there were no significant differences on with CA, PS, H dialyzing.
