尿毒症血液透析患者C反应蛋白改变及相关临床意义

摘要: 【摘要】目的 探讨尿毒症血液透析患者C反应蛋白的改变原因以及炎症反应与营养不良和贫血的关系。方法 抽取31例尿毒症血液透析患者血液透析前静脉血以及15例健康对照组静脉血均检测其C反应蛋白(CRP),血肌酣(SCr),血清白蛋白(Alb), 血红蛋白(Hb), 血清铁蛋白(SF)以及血液透析后静脉血检测其CRP,并根据血液透析前CRP水平将其分为正常组(CRP<10mg/L),异常组(CRP>10mg/L)。结果 尿毒症血液透析患者CRP正常组与异常组Alb,Hb统计学上有显著性差异(P<0.01),而两组的SCr、SF则没有显著性差异(P>0.05),单次血液透析后血浆CRP水平明显升高,血液透析前和血液透析后统计学上有显著性差异(P<0.01),尿毒症血液透析患者C反应蛋白与其 Alb,Hb成负相关(P<0.01),与SCr、SF无相关关系(P>0.05)。结论 血液透析本身的因素是导致尿毒症血液透析患者发生炎症反应使C反应蛋白升高的重要原因,而炎症反应在尿毒症血液透析患者营养不良和贫血中起着重要作用。

Abstract:

【Abstract】 Objective To explore the cause of changes in serum c-reactive protein(CRP) and inflammation in the malnutrition and anemia of uremic patients with hemodialysis(HD). Methods Detect 31patients with HD and 15 normal controls CRP、SCr、Alb、Hb、SF and CRP after HD, uremic patients with were divided into normal group (CRP<10mg/L), and abnormal group(CRP>10mg/L). Results Normal group with the abnormal group,Alb、Hb were significant difference was observed between groups(P<0.01), CRP increase significantly after HD compared with that before HD, CRP had significant difference in patients before and after HD(CRP<0.01).In patients of normal group and abnormal group,negative correlations of CRP with Alb、Hb were found.CRP was not found correlated with SCr and SF. Conclusions HD itself and individual is an important factor to increase CRP of serum patients with HD. Inflammation has an important role in the malnutrition and anemia of uremic patients with HD.
