晚期蛋白质氧化产物(AOPP)对血液透析患者单核细胞趋化因子受体CXCR2的影响

摘要:

目的 探讨不同透析膜对维持性血液透析(MHD)患者晚期蛋白质氧化产物(AOPP)及单核细胞趋化因子受体CXCR2的影响。方法 通过高效液相色谱法和流式细胞术测定长期采用铜仿膜(CU)和聚砜膜(PSU)透析患者外周血晚期蛋白质氧化产物(AOPP)及单核细胞趋化因子受体CXCR2表达。结果 MHD患者血浆 AOPP 水平明显增加(P<0.05),其中CU 组为(81.7±7.1) ng/ml;PSU 组为 64.5±7.3)ng/ml,与正常对照组(49.3±6.1)ng/ml相比均有显著差异(P<0.01,P<0.05);而PSU组与CU组相比,外周血AOPP 水平明显下降(P<0.05)。CXCR2在正常人外周血单核细胞中无表达,而在尿毒症未透析组及血液透析组均有表达,其中未透析组表达较低,而血液透析组表达较高,CU膜血液透析组较PSU血液透析组表达要高( P<0.05)。经相关分析结果显示:CU组与PSU组内AOPP水平与CXCR2表达均呈现良好的相关性(r=0.6409与0.583,P<0.05)。结论 血液透析通过血液-透析膜间相互反应,导致细胞内环境处于“氧化应激”状态,外周血 AOPP水平明显增高;而增高的AOPP水平可活化单核细胞,表达CXCR2增多。然而不同透析膜对MHD患者AOPP及单核细胞趋化因子受体CXCR2的影响有所不同,这些变化可能与透析膜的生物相容性有关。

Abstract:

Objective To investigate the effect of different dialysis membranes on perepheral blood advanced oxidation protein products (AOPP) and the expression of monocyte chemokine receptor CXCR2 in maintenance hemodialysis patients(MHD). Methods The levels of perepheral blood AOPP and the expression of CXCR2 were measured by high-performance liquid chromatography and flow cytometry in the patients using dialysis membranes like cuprophone(CU)and polysulfone(PSU). Results As compared with normal control group AOPP which was (49.3±6.1) ng/mL, the levels of AOPP in MHD patients were significantly increased, but the levels of AOPP in PSU group which was (64.5±7.3) ng/mL were significantly decreased than that in CU group which is (81.7±7.1) ng/mL (P<0.01). The activity of CXCR2 was not expressed in the perepheral blood mononuclear cell of normal person, but do in the hemodialysis and non-dialysis group of uremia.CXCR2 activity was lower in the non-dialysis group, while was higher in the dialysis group,which in CU group was significantly increased than that in PSU group( P<0.05). There is a positive correlation between AOPP levels and CXCR2 expression in CU group and PSU group. Conclusion Hemodialysis could activate the cellular environment into oxidative stress condition through the reaction between blood and dialysis membrane,and could induce perepheral blood mononuclear cells to synthesize more AOPP, while abnormally increased AOPP would increase the expression of chemokine receptor CXCR2 on monocyte; But different dialysis membranes, such as CU and PSU, would have different effects on AOPP and CXCR2 in the MHD patients.All of these changes are associated with the dialysis membrane's biocompatibility.
