血液透析患者细胞凋亡抑制基因BcL-2表达对外周血淋巴细胞凋亡的影响

摘要:

目的 探讨细胞凋亡抑制基因BcL-2表达对血液透析(HD)患者外周血淋巴细胞凋亡的影响。方法 采用荧光双标单抗免疫标记淋巴细胞法和流式细胞术检测血液透析患者外周血淋巴细胞BcL-2表达水平。结果 ①血液透析患者的细胞凋亡抑制基因(BcL-2)表达较正常人明显降低(P<0.01), HD后,BcL-2表达白细胞介素2水平有所增加,并与白细胞介素2(IL-2)浓度变化呈显著正相关,而与白细胞介素2可溶性受体(sIL-2R)水平呈负相关; ②与正常对照组相比,血液透析患者活化诱导的CD3+细胞凋亡增强,且随着透析时间的延长,其凋亡率逐渐增加,呈现时间依赖性; ③在体外培养的淋巴细胞上清液中,血液透析患者的sIL-2R对外源性重组白细胞介素2(rIL-2)的反应性较正常人增高,细胞表面IL-2R的自发表达亦较正常组明显升高(P<0.01),而体内实验表明,血浆 sIL-2R变化与血肌酐水平呈显著正相关,并与透析时间有关。结论 BcL-2和IL-2表达异常可促使外周血淋巴细胞凋亡,并与血液透析患者免疫损伤的发生、发展相关。

Abstract: Objective To study the expression of BcL-2 and IL-2 on peripheral blood lymphocytes in the patients with Hemodialysis(HD), and the relationship between these changes with the impaired immunity in longterm hemodialysis patients. Methods Twenty-seven hemodialysis patients and 25 normal controls were investigated. The expression of CD3-FITC/BcL-2-PE and IL-2R on peripheral blood lymphocytes (PBL) were detected by flow cytometry.The contents of sIL-2R were measured by ELISA method. Pripheral blood samples were collected before and right after hemodialysis. Results ①The activation-induced cell death occured in the hemodialysis patients; ②The Bcl-2 expression of HD patients before HD was(11.2±0.79)% witch is significantly lower than that of normal controls(38.94 ±0.76)%, P<0.01, the BcL-2 level of the patients increased lightly after hemodialysis; ③The changes of cell factors: The IL-2R and sIL-2R level were (9.78±1.21)% and (813.15±134.78)% respectively, both were significantly higher than that of normal controls (P<0.01), After hemodialysis,the level of IL-2R decreased,while sIL-2R was still 3 times higher than the normal range.The content of IL-2 was (54.21±31.5)ku/L before and (68.76±27.55)ku/L after hemodialysis (P<0.05), the changes of sIL-2R were significantly positively related to the Scr level (P<0.05), and the changes of Bcl-2 was significantly positively related to IL-2 level. Conclusion The aberration of Bcl-2 and IL-2 expression may accelerate apoptosis of HD patients' PBL. It is involved in the pathogenesis of immuno-suppression.
