连续性血液滤过对老年性瓣膜病并发顽固性心力衰竭的治疗作用

摘要: 目的 探讨连续性静脉-静脉血液滤过(CVVH)对老年性瓣膜病并发顽固性心力衰竭的治疗效 果,寻求抢救和治疗该类疾病更迅速而有效的治疗手段。方法 对11例经内科药物治疗效果不佳的老年性瓣膜病并发顽固性心力衰竭患者给予CVVH治疗,观察治疗前后的临床表现、血液生化及神经内分泌指标。结果 CVVH可快速缓解心力衰竭的临床症状、纠正代谢紊乱,患者的死亡率由97.61%下降到63.64%( P<0.05), CVVH对神经内分泌指标的影响较小。结论 CVVH对老年性瓣膜病并发顽固性心力衰竭患者是一种迅速而有效的治疗方法,可在短时间内缓解临床症状,阻断疾病的恶性循环,为进一步治疗赢得宝贵的时间。

Abstract: Objective To explore the therapeutic effect of continuous venovenous hemofiltration on refractory heart failure in patients with senile valvular heart disease.Methods Clinical manifestation and changes of blood biochemistry,neuroendocrine index were investigated before and after continuous venovenous hemofiltration in patients with secondary to senile valvular heart disease refractory heart failure. Results The study showed continuous venovenous hemofiltration on refractory heart failure in patients with senile valvular heart disease could remarkably reduce mortality from 97.61% to 63.64% (P<0.05). Continuous Venovenous hemofiltration could improve and correct deteriorative plasma levels of sodium (Na), potassium (K), chlorine ion (Cl),blood urea nitrogen(Bun),creatinine(Cr) and albumin within short period,but could not improve neuroendocrine changes of patients with refractory heart failure. Conclution Our data shows that continuous venovenous hemofiltration could be a rapid and effective method on treating refractory heart failure in patients with senile valvular heart disease,it could relieve clinical symptoms,break vicious circle within short period and gain time for further treatment. 
