腹膜透析患者水盐清除与高血压的关系
目的 探讨腹膜透析患者水盐清除量和高血压的关系。方法 将我院170例腹膜透析水肿程 度不同的患者,分为3组:①无水肿组(n=65);②轻度水肿组(n=80);③中度水肿组(n=25)。测定患者的总液体和钠的清除,以及测定患者的细胞外液(ECW)、细胞内液(ICW)、总体液量(TBW)、及相关资料。结果 3组患者水盐的排出和血压有统计学差异:水肿程度越重的患者,其总液体的清除和总钠的清除量越多,血压越高ICW和TBW越高。结论 本研究提示腹膜透析患者更多的水盐清除并不说明更好的血压控制,控制腹膜透析透析患者水盐摄入尤为重要。
Objective To study the relationship between hypertension and fluid removal in peritoneal dialysis (PD) patients. Methods In this cross-sectional study, 170 stable PD patients in our center were studied. According to the degree of peripheral edema, they were divided into three groups: group 1 without edema (n=65), group 2 with mild edema (n=80) and group 3 with moderate edema (n=25). The total fluid and sodium removal were measured. The bioimpedance analysis was employed to assess the extracellular water (ECW), intracellular water (ICW) and total body water (TBW). Result The difference of total fluid and sodium removal, blood pressure were statistically significant between the three groups: the more severe the peripheral edema, the higher the total fluid and sodium removal, the higher blood pressure, the higher ECW and TBW. Conclusion This study showed that PD patients with more fluid and sodium removal was not necessarily related to good blood pressure control, which indicated that fluid and sodium restriction was important in control hypertension in PD patients.
