摘要
目的 观察应用Fogarty取栓导管处理动静脉内瘘血栓形成的效果。方法 切开瘘管，以F3导管在球囊萎瘪的状态下轻柔地插入血管腔，导管穿过血栓后，注入肝素盐水使导管球囊向近侧插入，然后充起球囊导管，逐渐拉出导管，使血栓或栓子一起摘除。结果 Fogarty球囊导管取栓术18例，15例使用效果良好，血流量可达250ml/min以上，完全可满足血液透析需要，3例患者透析血流量不足,6例患者重新栓塞，12例长期使用，最长26月。结论 Fogarty取栓导管处理动静脉内瘘血栓形成，效果令人满意，尤其是有利于保存患者的原瘘管血管，延长瘘管使用寿命。
Abstract
Objective To evaluate the effect of Fogarty catheters in removing dialysis vascular access thrombus. Methods The thrombosed vascular access was incised and 3 Fr catheter was inserted. After the Fogarty catheter broke through the thrombus, the saline was infused into the balloon and then the catheter was pulled back. Results The operations were performed in 18 patients. All of them received hemodialysis within 24h. The access flow was restored and allowed successful hemodialysis with blood flow 250 ml/min in 15 patients, 3 patients had poor blood flow. The fistulas rethrombosed in 6 patients. Twelve patients vascular accesses were used well and the longest using time was 26 months. Conclusions To remove the vascular access thrombus by using the Fogarty catheter could get a satisfied results which can reserv the same vascular access and prolonge the vascular access life.
