不同血液净化方式与尿毒症患者生存质量的关系

摘要： 目的 研究血液透析滤过(HDF)和血液透析串联血液灌流(HD+HP)对尿毒症患者生存质量的影响。方法 选择2005年2～8月哈尔滨医科大学附属二院肾内科血液净化中心长期规律血液透析患者20例，随机加做血液透析滤过和(或)血液透析串联血液灌流，时间至少3个月，进行前后两次SF-36问卷调查。结果 ① SF-36总分、生理健康总分、躯体疼痛（bodily pain，BP）、躯体角色功能（role-physical，RP）、总体健康（general health，GH）维度在应用HDF和(或)HD+HP后明显高于应用前，差异有显著性；②而心理健康总分、躯体健康（physical functioning，PF）、情绪角色功能（role-emotional，RE）、心理健康（mental health，MH）、精力（vitality，VT）维度在应用前后差异无显著性；③社会功能（social functioning，SF）维度在应用后明显低于应用前，差异有显著性。 结论 ① 应用HDF和(或)HD+HP可以明显改善尿毒症患者的生存质量，尤其在改善躯体疼痛方面；② 尿毒症患者的心理健康并没有因为加做HDF和(或)HD+HP而得到改善。

Abstract:

Objective To assess the effect of hemodiafiltration and combined hemodialysis- hemoperfusion on the quality of life in patiens with uremia. Methods Twenty maintenance hemodialysis patients who were randomized to receive HDF and/or HD-HP therapy more than three months were enrolled in the study. The quality of life was evaluated using SF-36 questionaire. Results ①Total score of SF-36, total score of physiohealth, bodily pain, role-physical and general health score significantly rise in uremic patients after the administration of HDF and/or HD-HP; ②Total score of psychohealth, physical functioning, role-emotional, mental health and vitality had no evident change in uremic patients after HDF and/or HD-HP; ③ social functioning score significantly decreased in uremia patients after HDF and/or HD-HP treatment. Conclusion ① HDF and/or HD-HP may significantly improve the quality of life of uremic patients, especially release bodily pain; ② But the two approaches may not improve the psychohealth of uremic patients .
