高通量血液透析和血液透析滤过对血清 2-微球蛋白的影响

摘要: 目的 比较低通量血液透析(HD)、高通量血液透析(HFHD)和血液透析滤过(HDF)3种透析方式对维持性血液透析患者尿素氮、肌酐、血磷和β2-微球蛋白清除的影响。 方法 选择北京朝阳医院血液净化中心70例维持性血液透析患者随机分为3组,分别为HD组28例, HFHD组20例和HDF组22例。在透析前后分别取血检测患者尿素氮、肌酐、钙、磷和β2-微球蛋白水平。结果 3组患者透析前后的尿素氮、肌酐和血磷水平均明显下降,其中HDF组降低更为明显。HFHD组和HDF组患者循环中的β2-微球蛋白透析后显著下降,而HD组透析前后β2-微球蛋白差异没有显著性。结论 HD、HFD、HDF均能有效清除尿毒症患者血液中的小分子溶质,HFHD和HDF对β2-微球蛋白的清除能力明显优于HD。

Abstract:

Objective To compare the effects of different dialysis modes, high-flux hemodialysis, on-line hemdiafiltration and low-flux hemodialysis on serum β2- microglobulin. Methods Seventy chronic dialysis patients were randomized to receire low-flux hemodialysis (HD n=28), high-flux hemodialysis (HFHD n=20) and on-line hemodia filtration(HDF n=22).The serum blood urea nitrogen, creatinine, phosphorus and β2- microglobulin were examined pre and post dialysis. Results In the HDF group, the reduction ratio of serum blood urea nitrogen, creatinine and phosphorus was higher than that in the other groups. The concentration of β2-microglobulin significantly reduced after the dialysis session in the HFHD group and the HDF group while no significance was found in the HD group. Conclusion HFHD and HDF could efficiently eliminate both the low molecular weight solutes and β2- microglobulin. HFHD and HDF should be applied to remove the β2-microglobulin in the maintenance hemodialysis patients.
