阿魏酸哌嗪对血液透析患者残余肾功能的影响

摘要:

目的 探讨中药阿魏酸哌嗪对维持性血液透析( MHD)尿毒症患者残余肾功能(RRF)的影响。方 法 32例MHD的尿毒症患者,随机分为治疗组和对照组,每组各16例。治疗组在对照组治疗的基础上加用中药制剂-阿魏酸哌嗪,每次5片(每片含阿魏酸哌嗪50mg),每日3次口服,疗程24个月。观察并记录治疗前、后患者的一般状况、血压(BP)、体重(BW)、血红蛋白(Hb)、血尿素氮(BUN)、肌酐(Scr)、白蛋白(Alb)、前白蛋白(PA)等指标;分别检测两组患者透析前及透析后6、12、24个月的RRF及尿量。结果 与对照相比,治疗组治疗后RRF下降的速度明显减慢( P<0.05),尿量减少的幅度明显降低(P<0.01), Hb、BW、Alb、PA明显高于对照组(P<0.05~0.01), BP较对照组控制的更满意(P<0.05)。对照组治疗后 Alb、PA数值较治疗前无显著性差异异。结论 阿魏酸哌嗪能延缓MHD的尿毒症患者RRF丢失及尿量减少的速度,改善其营养状况,提高生活质量。

Abstract:

Objective To investigate the effect of Piperazine Ferulate on residual renal function(RRF) in hemodialysis(HD) patients. Methods Thirty-two maintenance hemodialysis(MHD) patients of uremia were divided into two groups randomly. Sixteen patients in the treated group and sixteen in the control group. All patients were treated by bicarbonate HD. The blood and dialysate flow, treating time, dialyser and recursive times were the same, but the patients of the treated group combined with Piperazine Ferulate 5 tablets (each tablet contains Piperazine Ferulate 50mg) 3 times per day. The therapeutic course were 24 months. The changes in general condition, blood pressure(BP), body weight(BW), hemoglobin(Hb), blood urea nitrogen(BUN), serum creatinine (Scr), albumin(Alb), pre-albumin(PA) before and after treatment were observed. RRF and amount of urine were measured before and after 6,12,24 months' treatment respectively. Results In the treated group ,after treatment, the decline of RRF and amount of urine were significantly slower than those in the control group (P<0.05~0.01), significant improvement were revealed in aspects of Hb, BW, Alb, PA in the treated group, as compared with those in the control group (P<0.05~0.01).Also the levels of BP in former were markedly better than later group (P<0.05).No significant changes of Alb and PA were observed in the control group. Conclusion Piperazine Ferulate was effective in slowing down the decrease of RRF and amount of urine, improving nutritional condition and quality of life in patients of uremia treated by HD.
