维持性血液透析对尿毒症患者血脂水平影响及相关分析

摘要：

目的 观察尿毒症患者行血液透析治疗前、后的血脂水平的变化,并对相关因素加以探讨。方法 血液透析治疗组28例,正常对照组26例，所有患者均在治疗前及治疗12个月后测定血浆胆固醇(TCH)、甘油三酯(TG)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、载脂蛋白A1(ApOA1)、载脂蛋白B(ApOB)及血浆白蛋白(ALB)、血肌酐(Scr) 。结果 尿毒症血液透析患者TCH、TG、LDL、ApOB与正常对照组比较显著上升( P＜0.05)，而 HDL和ApOA1与正常对照组明显下降 P＜0.05)，透析患者前、后比较无显著性差异(P＞0.05)，透析组血浆白蛋白与血脂水平呈显著相关。结论 尿毒症患者在透析前即存在脂代谢异常，透析治疗不能消除高脂血症，且加重脂代谢紊乱，同时血液透析治疗血浆白蛋白与血脂水平呈显著相关表明低蛋白血症，也是高脂血症致病原因之一。

Abstract:

Objective To study the effects of hemodialysis on lipidemia in patients with end stage renal disease(ESRD). Methods TCH、TG、LDL、ApOA1、ApOB、ALB、Scr of the study population which included 26 healthy subjects and 28 patients with end stage renal disease undergoing hemodialysis were detected. Results There were significant difference of serum TCH, TG, HDL, ApOA, LDL AND ApOB, between ESRD patients and normal controls (P<0.05). The serum TCH, TG, LDL and ApOB of ESRD patients were higher than normal controls yet HDL and ApOA, were lower. There were no significant difference between before and after hemodialysis(P>0.05). Conclusion The hyperlipide-mia in ESRD patients conldn't be improved by hemodialysis. Hypoalbuminemia is correlated with hyperlipidemia.
