透析液流量、血液流量对空心纤维透析器面积相关溶质转运系数的影响

摘要: 【摘要】目的 评价透析液流量、血液流量对空心纤维透析器面积相关溶质转运系数(mass transfer area coefficient,KoA)的影响。方法 体外条件下,对两种透析器、9组不同透析液流量和血流量组合的尿素KoA进行了测定。 结果 透析液流量(Qd)保持恒定时(分别为500,800,1000 ml/min),尿素KoA在血流量从300增加到400,500 ml/min时,KoA并不增加;但在血流量(Qb)保持恒定时 (分别为300,400,500 ml/min),尿素KoA在透析液流量从500增加到800,1000ml/min时,尿素KoA显著增加。 结论 KoA随Qd 的增加而增加, 而增加Qb对透析器的KoA影响较小。将Qd从500ml/min 提高至800ml/min是对部分患者使用其它方法难以获得满意的透析剂量时值得考虑的方法。

Abstract: 【Abstract】Objective The purpose of this study was to measure, in commercially available hemodialyzers, the spatial variation of the mass transfer area coefficient (KoA) for urea and to evaluate its dependence on dialysate flow rate (Qd) and blood flow rate (Qb).Methods Two different models of hollow fiber dialyzer were used in vitro for nine countercurrent Qb and Qd combinations. Results KoA for urea varied with dialyzer model, but the effect of Qb, Qd. Urea KoA did not change significantly when Qb increased from 300 to 500 ml/min at Qd of 500, 800 and 1000 ml/min. When Qd increased from 500 to 1000 ml /min at a Qb between 300 and 500 ml/min, however, urea KoA increased significantly. Conclusion These data demonstrate that increasing Qd alters the mass transfer characteristics of hollow fiber dialyzers and results in increased urea clearance.
