维持性血液透析老年患者血容量监测的意义

摘要： 目的 观察血液透析过程中血容量监测对老年透析患者的意义。方法 对44例患者进行122 次实时动态血容量监测，同时观察容量下降值ΔBV与超滤量及心率、血压、抽搐等并发症的发生率。结果 在超滤量大致相同时，血容量下降值ΔBV无显著差异，老年患者血液透析并发症发生率高，特别是伴有心脏功能不良的老年透析患者，并发症明显增加。结论 老年透析患者在透析中进行血容量监测十分重要，伴有心脏功能不良的老年患者尤其重要。

Abstract: Objective To discuss the significance of blood volume monitor during the hemodialysis to the old patients. Methods Fourty-four patients who took hemodialysis were selected to take blood volume monitor for 122 times (young patients 54 times , old patients accompany with heart malfunction 40 times, old patients without heart malfunction 28 times), then observe volume descent titer ΔBV、ultrafiltration dose、heart rate、blood pressure and the incidence of complications, such as tic simultaneously. Results Under almost identical ultrafiltration dose, there aren't any significant differences in blood volume descent titer ΔBV among three teams. To contrast to the young patient team, the incidence of emodialysis complication is higher in old patient teams, particularly to old patients who accompany with heart malfunction, the incidence of complication increased significantly. Conclusions It's very important for old patients to take blood volume monitor during hemodialysis, especially for those who accompany with heart malfunction.
