血浆置换治疗伴呼吸肌无力危象的格林-巴利综合征和重症肌无力疗效观察

摘要: 目的 观察血浆置换治疗伴呼吸肌无力危象的格林-巴利综合征(GBS)和重症肌无力(MG)患者的效果。方法 选择中山大学附属第一医院肾内科32例伴呼吸肌无力危象的GBS和MG患者中,15例采用单纯药物治疗,17例联用血浆置换,比较两组患者呼吸机辅助呼吸和住院时间。 结果 血浆置换治疗组和单纯药物组呼吸机辅助呼吸时间分别为(8.7±8.4)d、(19.8±15.0)d,住院时间分别为(33.3±21.9)d、(52.7±28.4)d,两组比较差异有统计学意义(P<0.05)。 结论 血浆置换可有效缩短伴呼吸肌无力危象的GBS和MG患者呼吸机辅助呼吸和住院时间,值得临床推广应用。

Abstract: Objective To observe the effect of plasmapheresis in treatment of respiratory muscular paralysis crisis in patients with Guillain-Barre syndrome and myasthenia gravis. Methods Among the 32 patients showing respiratory muscular paralysis crisis complicated in Guillain-Barre syndrome or myasthenia gravis, 15 patients received drug treatment, and 17 patients received drug plus plasmapheresis treatment. We compared mechanical ventilation and hospitalization days between the two groups. Results The period of using mechanical ventilation was 8.7 8.4 days in drug plus plasmaphresis group and was 19.8 15.0 days in drug group (P<0.05). Hospitalization time was 33.3 21.9 days in drug plus plasmaphresis group and was 52.7 28.4 days in drug group (P<0.05). Conclusions Plasmapheresis reduces the time of using mechanical ventilation and hospitalization day in respiratory muscular paralysis crisis occurring in patients of Guillain- Barre syndrome or myasthenia gravis.
