ABC 情绪疗法改善血液透析患者的负性情绪及营养不良的临床研究

Clinical research of ABC emotional therapy on negative emotion and malnutrition in hemodialysis patients

摘要: 【摘要】目的探讨艾利斯ABC 情绪疗法改善血液透析患者的不良情绪及营养不良的效果。方法本研究纳入152 例维持性血液透析患者,随机分为2 组,每组各76 例。其中对照组采取常规护理方法,实验组采用常规护理和艾利斯ABC 情绪疗法。比较2 组的焦虑、抑郁,食欲,营养状态及患者生活质量的变化情况。结果入组前,2 组的焦虑自评量表(self-rating anxiety acale, SAS)、抑郁自评量表(self-rating depression scale,SDS)和生活质量评分均无明显差异。入组后第3 个月,实验组的SAS、SDS 评分均显著低于对照组(t 值分别为-1.972,-1.982;P 值分别为0.049,0.039);入组后6 月和12 月,实验组的食欲量表(council on nutrition appetite questionnaire,CNAQ)评分和营养不良炎症评分(malnutrition inflammation score,MIS)评分明显低于对照组(t 值分别为1.976,1.996;P 值分别为0.041,0.021)。入组后第6 个月实验组的生活质量评分量表中躯体功生理功能、生理职能、情感职能分值均显著高于对照组(t 值分别为2.357,2.982,2.181;P 值分别为0.021,<0.001,0.012)。入组后第12 个月实验组的SF-36 生活质量量表中躯体功能、角色限制、躯体疼痛、健康总体自评、活力、情感角色分值以及SAS、SDS 评分均显著高于对照组(t 值分别为2.901,-2,023,2.721,2.981,2.324,2.181,-2.935,-3.121;P 值分别为<0.001,<0.001,0.011,0.021,0.031,0.013,0.001,0.023)。结论ABC 情绪疗法能够有效改善血液透析患者负性情绪和营养状态,提高患者生存质量。

Abstract: 【Abstract】Objective To investigate the therapeutic effect of Ellis ABC emotional therapy on negative mood and malnutrition in hemodialysis patients. Methods This study enrolled 152 maintenance hemodialysis patients. They were randomly divided into two groups; patients (n=76) in the control group were treated with routine nursing and those (n=76) in the experimental group were treated with Ellis ABC emotional therapy in addition to routine nursing. Anxiety, depression, appetite, nutritional status and quality of life were compared between the two groups. Results There were no significant differences in self-rating depression scale (SDS) score and quality of life score between the two groups before the study. After the treatment for 3 months, self- rating anxiety scale (SAS) and SDS scores were significantly lower in the experimental group than in the control group (t=-1.972, P=0.049; t=-1.982, P=0.039). After the intervention for 6 and 12 months, the council on nutrition appetite questionnaire(CNAQ) and malnutrition inflammation score(MIS) scores were significantly lower in the experimental group than in the control group (t=1.976, P=0.041; t=1.996, P=0.021). After the intervention for 6 months, the scores of body physical function, physiological function and emotional function were significantly higher in the experimental group than in the control group (t=2.357, P=0.021; t=2.982, P<0.001; t=2.181, P=0.012). After the intervention for 12 months, the scores of body physical function, physiological function, somatic pain, general health, vitality, emotional functional domain, SAS and SDS were significantly higher in the experimental group than in the control group (t=2.901, -2,023, 2.721, 2.981, 2.324, 2.181, -2.935 and -3.121 respectively; P<0.001,<0.001, 0.011, 0.021, 0.031, 0.013, 0.001 and 0.023 respectively). Conclusion ABC emotional therapy can effectively improve the negative emotion, nutritional status and quality of life in hemodialysis patients.
