摘要: 1型心肾综合征(cardiorenal syndrome type 1,CRS-1)指急性心功能恶化导致急性肾损伤(acute kidney injury,AKI)的病生理综合征。多发生于急性失代偿性心力衰竭(acute decompensated heart failure, ADHF)。CRS-1发生发展和诊治中出现的AKI又称作肾功能恶化(worsening renal function,WRF)。本文就WRF的定义、诊断标准、发病机制、CRS-1中WRF的意义及病因分析,以及WRF治疗策略等方面进行综述,旨在提高对CRS-1中AKI的识别和病生理机制理解,为提高CRS-1救治水平、改善预后提供理论依据。

Abstract: Cardiorenal syndrome type 1 (CRS-1) refers to the pathophysiological syndrome of acute kidney injury (AKI) caused by acute deterioration of cardiac function. It is more common in acute decompensated heart failure (ADHF). AKI in the development of CRS-1 is also known as worsening renal function (WRF). This article reviews the definition, diagnostic criteria, pathogenesis, clinical significance and etiology of WRF in CRS-1, as well as therapeutic strategies for WRF. The aim is to enhance recognition of AKI in CRS-1 and deepen understanding of its pathophysiology, thereby providing a theoretical basis for improving CRS-1 management and patient prognosis.
