干扰素-α对高糖环境下腹膜间皮细胞合成细胞外基质的影响

摘要: 目的 通过研究干扰素-α(IFN-α)对高糖环境下腹膜间皮细胞 (HPMCs) 分泌纤维连结蛋白(FN)、转化生长因子-?(TGF-?)、基质金属蛋白酶2(MMP-2)的影响,探讨其对腹膜透析相关性腹膜纤维化的防治意义。方法 收集行不卧床持续腹膜透析(CAPD)尿毒症患者腹膜透出液中的腹膜间皮细胞,进行原代培养,传代细胞用于实验。设实验组和对照组,实验组加2.5%的葡萄糖及不同剂量的干扰素,培养不同时间分别收集上清液,用ELISA法检测其中的FN、TGF -?、MMP-2含量,并进行统计学分析。结果 各加2.5%葡萄糖的实验组FN含量及TGF-?含量均高于对照组,差异具有显著性(P<0.05);加干扰素的实验组FN及TGF-? 含量均低于不加干扰素的实验组,差异具有浓度依赖性,浓度为1 000u时达到高峰;MMP-2含量: 对照组与实验组间无显著性差异。结论 高浓度葡萄糖可促进腹膜间皮细胞表达FN、TGF-?,但对MMP-2含量无显著影响。而干扰素对FN、TGF-?具有相反的作用,提示其可能减少CAPD时细胞外基质的堆积,对预防、延缓腹膜纤维化有一定意义。

Abstract: Objective Human peritoneal mesothelial cells(HPMCs)are important components of the peritoneum and play a very important role in peritoneal dialysis (PD) and peritoneal fibrosis caused by peritoneal dialysis. The physio-morphological alteration of PD related peritoneal fibrosis is marked by the loss of peritoneal mesothelial cell layer and the accumulation of extracellular matrix (ECM). And during the two processes, HPMCs are not only passive sufferers, but also active participants. In the incompatible physiological environment induced by the peritoneal dialysate, HPMCs are injured and their proliferations are suppressed. They also secret ECM and ECM accumulating cytokines including the most important cytokine among them: transforming growth factor- α1 (TGF-α1).In this study, we investigated the effect of interferon-α(IFN-α) on the secretion of fibronectin (FN)、transforming growth factor-β1 (TGF-β1)、matrix metalloproteinases (MMP-2) in human peritoneal mesothelial cells (HPMCs) under the condition of high glucose concentration trying to understand the significance of IFN-αon the prevention and treating of peritoneal fibrosis caused by peritoneal dialysis. Methods HPMCs were isolated from discarded peritoneal dialysate which comed from the patients who were receiving continuous ambulatory peritoneal dialysis, and primarily cultured. The secondarily cultured HPMCs were used in the experiment. We can make cellular evaluation based on observation of the cells by light micros-cope and based on staining of immuno-histochemical methed with anti-body for vimentin, cytokeratin8, factor Ⅷ. HPMCs were divided into experi-mental groups and control group. The experimental groups were treated with 2.5% glucose and IFN-αof different concentrations. The super-nate was collected after 24h, 48h and 72h to detect FN、TGF-β1、MMP-2 in it. Results 1.HPMCs: The cultured HPMCs were spindly at first. About a week later, they connected with each other and took on a slabstone-like appearance. Immunohisto-chemistry analysis showed positive immunoreactivity of vimentin and cytokeratin and negative immunoreactivity of factor Ⅷ in these cells. 2. The FN-like and the TGF-β1-like immunoreactivity in all the experimental groups treated with 2.5% glucose was significanly higher than that in the control group (P <0.05). The FN-like and the TGF-β1-like immunoreactivity in groups treated with 2.5% glucose and IFN-αwas significantly lower than that in groups treated with 2.5% glucose only (P<0.05); IFN-α reduced the synthesis of FN TGF-β1 in a dose-dependent manner. MMP-2: There is no marked difference between control group and experiment groups. Conclusions Glucose of high concentration can promote the synthesis of FN、TGF-β1 but has no effect on the synthesis of MMP-2 in HPMCs. IFN-αcan reverse these changes. IFN-αis likely to reduce the accumulation of ECM in CAPD and it has certain significance in the prevention and treating of peritoneal fibrosis.
