再培训对患者腹膜透析操作技术的影响

目的 研究再培训对患者腹膜透析操作技术的影响,为今后的门诊再培训提供依据。 方法 对2006年3月至2009年2月期间因尿毒症行腹膜透析治疗的75例患者,进行透析操作9个环节的考核,随即根据考核情况进行再培训,6~12月后再进行第2次操作考核。比较2次透析操作考核的情况,并分析影响再培训效果的影响因素。 结果 75例腹膜透析患者的平均年龄(57.4±14.7)岁,男性27例,女性48例。经再培训后,操作考核评估显示达标率从13%上升到24%,差异无统计学意义(P>0.05);但总错误项自(2.6±1.7)项减少到(1.8±1.5)项,差异有统计学意义(P<0.05)。再培训后操作技术改善组的年龄明显小于不变与变差组,分别为(48.2±16.3)岁和(57.4±10.8)岁,差异有统计学意义(P=0.045)。 结论 再培训后腹膜透析患者的透析操作技术明显改善。年纪越大的患者再培训效果越差。

Abstract:

Objective To explore the impact of retraining on bag exchange technique, and to provide the evidences for the retraining in future. Methods The bag exchange techniques including 9 items were examined in 75 patients on peritoneal dialysis between Mar. 2003 and Feb. 2009. Retraining was performed based on the results of this examination. Then the second examination for bag exchange techniques was implemented after 6-12 months of retraining. The two examinations of bag exchange were compared, and the related factors for the effect of retraining were analyzed. Results The mean age was 57.4±14.7y in the 75 patients, with 27 males and 48 females. After retraining to the 75 patients, the correct rate of bag exchange numerically increased from 13% to 24%. The total incorrect items decreased from 2.6±1.7 items to 1.8±1.5 items, achieving a statistical significance. The patients improved in bag exchange technique were significantly younger than those not improved after retraining (48.2±16.3yrs vs. 57.4±10.8yrs, P=0.045). Conclusion The bag exchange technique could be significantly improved by the retraining. Retraining seems to have better results in younger patients on peritoneal dialysis.
